The Northeast Conference Request for Reimbursement Form has been updated...
The form now reflects mileage reimbursement in the amount of 25 cents per mile which is a match to the Indiana-Kentucky Synod mileage reimbursement policy of paying 25 cents per mile. If you have downloaded this form prior to Sep 29, 2008, please destroy older copies and download a current copy.
